How information is used
Information is used to validate the request, contact you about it, work with a dental practice to arrange care, prevent spam and duplicates, document consent, and operate the service.
When information is shared
Request information is received by AIXIS, LLC d/b/a Carbide Digital and may be shared with a licensed Vermont dental practice in order to arrange your care, and with service providers that support secure hosting, form delivery, communications, or recordkeeping under appropriate agreements. It is not sold for unrelated advertising.
Retention and choices
Appointment-request records are generally retained for up to 24 months after the most recent activity, then deleted or de-identified unless a longer period is reasonably needed for security, dispute, accounting, or legal obligations. Email team@carbidedigital.io to request access, correction, deletion, or withdrawal of communication consent. We may verify identity before completing a request.
Website operator
AIXIS, LLC d/b/a Carbide Digital controls the website appointment-request record. If the request is shared with a dental practice, that practice applies its own privacy notice to clinical intake and care records.
Health privacy
Whether HIPAA applies depends on the parties and data flow. The service does not claim HIPAA compliance without a documented assessment. Sensitive clinical intake occurs through the participating practice's approved system.
